Anti–Intrinsic Factor (IF) IgG antibodies are autoantibodies directed against intrinsic factor, a glycoprotein secreted by gastric parietal cells that is essential for the absorption of vitamin B₁₂ (cobalamin) in the ileum. The presence of these antibodies interferes with the binding of vitamin B₁₂ to intrinsic factor or with the uptake of the IF–B₁₂ complex by intestinal receptors, leading to vitamin B₁₂ deficiency. This deficiency results in defective DNA synthesis, megaloblastic anemia, and neurological dysfunction. Anti-IF IgG is therefore a hallmark biomarker of pernicious anemia, an autoimmune gastritis characterized by the destruction of parietal cells and loss of intrinsic factor secretion.
In the clinical setting, detection of anti–intrinsic factor antibodies is used as a highly specific diagnostic marker for pernicious anemia and related causes of vitamin B₁₂ malabsorption. While not as sensitive as anti-parietal cell antibodies, anti-IF IgG testing provides strong diagnostic confirmation when positive, especially in patients presenting with macrocytic anemia, low serum B₁₂, or neurologic symptoms suggestive of deficiency. The assay helps distinguish autoimmune causes of B₁₂ deficiency from nutritional or malabsorptive etiologies and supports early treatment to prevent irreversible neurological damage.
In research applications, anti-IF IgG serves as a key biomarker in studies exploring autoimmune gastritis, vitamin B₁₂ metabolism, and immune tolerance within the gastric mucosa. It is also used to investigate the immunogenetic predisposition to pernicious anemia and its association with other autoimmune disorders such as thyroid disease and type 1 diabetes. Research on anti-IF antibodies contributes to understanding the mechanisms of organ-specific autoimmunity and to developing more sensitive diagnostic assays for early detection of autoimmune gastric diseases.
