Assay Background
The complement system is a proteolysis-based activation cascade which belongs to the innate immune response and acts as a first line defense against infections. It is involved in opsonization, promotes inflammation and enhances the adaptive immune response. The complement system can be activated through three distinct pathways, the classical (CP), the lectin (LP) and the alternative pathway (AP). Activation of all three pathways leads to the formation of a C3 and a C5 convertase, ultimately resulting in the formation of the membrane attack complex (MAC complex) and cell lysis. The lectin pathway is activated by binding of pattern recognition molecules mannan-binding lectin (MBL), collectins and ficolins to carbohydrate structures and acetylated residues on the surface of pathogens and altered cells. After binding, MASP-1 zymogen auto-activates and transactivates MASP-2. MASP-2 further cleaves the C4 complement component, whereas both MASP-1 and MASP-2 cleave C2. This leads to the formation of the C3 convertase (C4bC2b). The C3 convertase cleaves C3, leading to complement terminal pathway activation and thus mediating the destruction of pathogens or altered cells. C1 inhibitor (C1INH) is regulating lectin pathway activation by binding to either MASP-1 or MASP-2, leading to the formation of covalent MASP1/C1-INH and MASP2/C1-INH complexes. A given level of those complexes naturally exists in the circulation, but concentrations can change and lower levels of MASP1/C1-INH complex have for example been shown in Hereditary Angioedema (HAE) patients, when compared to healthy controls. The concentration of MASP1/C1-INH complex increases over time when the lectin pathway is activated in human serum, making the complex an indirect marker for ongoing lectin pathway activation.
