Anti-cardiolipin (aCL) IgG is an autoantibody directed against cardiolipin, a phospholipid involved in mitochondrial function and present on cell membranes, especially during apoptosis. It is one of the primary antibodies used in diagnosing antiphospholipid syndrome (APS), an autoimmune disorder characterized by recurrent arterial or venous thrombosis and pregnancy complications. Among the aCL isotypes, IgG is the most clinically significant due to its strong association with thrombotic events and adverse pregnancy outcomes.
In clinical settings, aCL IgG is routinely tested in patients with suspected APS, especially those with unexplained blood clots, recurrent miscarriage, or autoimmune diseases like systemic lupus erythematosus (SLE). It is measured via standardized ELISA, and persistent moderate-to-high titers—confirmed at least 12 weeks apart—are required for APS diagnosis under current classification criteria. In research, aCL IgG is extensively studied for its pathogenic role in thrombosis, its interaction with β2-glycoprotein I (a cofactor that enhances antigenicity), and its contribution to endothelial cell activation and complement-mediated injury. These investigations aim to refine diagnostic strategies and develop targeted therapies that interrupt antibody-driven vascular damage without broad immunosuppression.
