Anti-tissue transglutaminase (anti-tTG) IgG is an autoantibody directed against tissue transglutaminase, an enzyme that modifies gluten peptides and is central to the autoimmune response in celiac disease. While anti-tTG IgA is the preferred biomarker due to its higher sensitivity and specificity in individuals with normal immune function, anti-tTG IgG is particularly valuable in patients with selective IgA deficiency—a condition more common in those with celiac disease. In these individuals, IgG-based testing becomes essential for accurate diagnosis.
In clinical settings, anti-tTG IgG is used as a complementary test when total serum IgA is low or deficient. A positive anti-tTG IgG result in this context supports further diagnostic evaluation, such as intestinal biopsy or HLA typing. Though not as specific as its IgA counterpart, anti-tTG IgG can also help monitor disease activity and dietary compliance in certain cases.
