Anti-cyclic citrullinated peptide (anti-CCP or anti-CP) IgG antibodies are autoantibodies that target peptides containing citrulline, a modified amino acid formed by the post-translational deimination of arginine. These antibodies are highly specific for rheumatoid arthritis (RA), making anti-CCP IgG one of the most important serologic biomarkers for this disease. Citrullinated proteins are thought to become immunogenic in genetically susceptible individuals, leading to chronic joint inflammation and autoimmunity.
In clinical settings, anti-CP IgG is used primarily to aid in the diagnosis of RA, particularly in patients with early or undifferentiated arthritis. The presence of anti-CP IgG often precedes clinical symptoms and is associated with more aggressive disease, joint erosion, and poorer long-term outcomes. When combined with rheumatoid factor (RF), anti-CP IgG improves diagnostic sensitivity and specificity. In research, this biomarker is studied to understand the mechanisms of citrullination in autoimmunity, investigate environmental and genetic risk factors (e.g., smoking and HLA-DRB1), and stratify patients for targeted therapies. Anti-CP IgG is also used in clinical trials to evaluate treatment responses and predict disease progression.
