Anti-Gliadin IgG antibodies (AGA IgG) are autoantibodies directed against gliadin, a component of gluten found in wheat and related grains. Gliadin is a key immunogenic trigger in individuals with celiac disease (CD), an autoimmune enteropathy characterized by small intestinal villous atrophy, malabsorption, and systemic manifestations. Anti-gliadin IgG antibodies reflect immune reactivity to dietary gluten, particularly in patients who may be IgA-deficient or in cases where IgA-based testing is unreliable. These antibodies are part of the early humoral immune response and can appear before significant intestinal damage occurs.

In the clinical setting, anti-gliadin IgG testing is used primarily in screening for celiac disease, especially in patients with selective IgA deficiency, where IgA-based markers like anti-tTG IgA may be falsely negative. AGA IgG positivity supports the diagnosis of gluten sensitivity or celiac disease in the context of clinical symptoms such as diarrhea, weight loss, or growth failure in children. However, due to lower specificity compared to anti-tTG or anti-deamidated gliadin peptide (DGP) antibodies, AGA IgG is generally not used as a standalone diagnostic test but may complement other serologic assays.

In research, anti-gliadin IgG serves as a biomarker to study gluten-driven immune responses, non-celiac gluten sensitivity, and the pathophysiology of intestinal autoimmunity. It is often used in epidemiologic studies to identify populations with subclinical gluten sensitivity or early-stage celiac disease. Additionally, AGA IgG levels can be monitored to assess dietary compliance and immune response in individuals on a gluten-free diet, making it useful in both mechanistic studies and clinical trials related to gluten-associated disorders.
