Intact Parathyroid Hormone (PTH) is an 84–amino acid peptide secreted by the parathyroid glands that plays a key role in calcium and phosphate homeostasis. The “intact” PTH assay specifically measures the biologically active full-length molecule (PTH 1–84), distinguishing it from inactive fragments that circulate in the blood. This measurement provides a more accurate reflection of parathyroid gland function compared to older assays that detected both intact hormone and C-terminal fragments. Intact PTH secretion is tightly regulated by serum calcium levels via a negative feedback loop involving the calcium-sensing receptor (CaSR) on parathyroid cells.
