Anti–double-stranded DNA (anti-dsDNA) antibodies are autoantibodies that specifically target the double-stranded form of DNA, a key nuclear component. They are highly specific for systemic lupus erythematosus (SLE) and are considered a hallmark serologic biomarker of the disease. Anti-dsDNA antibodies are directly involved in the pathogenesis of SLE by forming immune complexes that deposit in tissues such as the kidneys, skin, and blood vessels, leading to inflammation and organ damage, particularly in lupus nephritis. The presence and titer of anti-dsDNA antibodies often correlate with disease activity, making them both diagnostic and prognostic markers.

In the clinical setting, anti-dsDNA testing is primarily used for diagnosis, disease monitoring, and prognostication in SLE. High titers, particularly of IgG anti-dsDNA, are strongly associated with active disease and renal involvement. Serial measurements can help track disease flares or remission and guide therapeutic decisions, especially in patients at risk for lupus nephritis. Anti-dsDNA antibodies are typically measured using ELISA, Farr assay, or Crithidia luciliae immunofluorescence, with each method offering different sensitivity and specificity profiles.
