Anti-beta-2 glycoprotein I (anti-β2GPI) IgM is an autoantibody targeting beta-2 glycoprotein I, a phospholipid-binding protein involved in coagulation and immune regulation. It is one of the three isotypes—IgG, IgM, and IgA—associated with antiphospholipid syndrome (APS), a disorder marked by recurrent thrombosis and pregnancy complications. While the IgG isotype is considered the most clinically relevant, the IgM form is included in the classification criteria for APS when persistently elevated and detected alongside compatible clinical features.
In clinical practice, anti-β2GPI IgM is tested as part of the antiphospholipid antibody panel, particularly in patients suspected of having APS. Its diagnostic value is generally considered lower than IgG, but it can be significant when present in isolation or in patients with incomplete clinical manifestations. Persistent positivity over time may support APS diagnosis, especially when accompanied by other autoimmune markers or risk factors. In research, anti-β2GPI IgM is investigated for its role in early immune responses, its potential contribution to thrombosis, and its interaction with complement and endothelial cells. Ongoing studies aim to clarify whether it has independent pathogenic significance or serves as a marker of broader immune dysregulation.
