Anti-beta-2 glycoprotein I (anti-β2GPI) IgG is a highly specific autoantibody directed against beta-2 glycoprotein I, a plasma protein that binds to negatively charged phospholipids and plays a role in coagulation homeostasis. This antibody is a key component of the classification criteria for antiphospholipid syndrome (APS), an autoimmune disorder characterized by recurrent arterial or venous thrombosis and pregnancy morbidity. Among the three isotypes—IgG, IgM, and IgA—the IgG isotype is the most clinically significant and strongly associated with thrombotic events and adverse pregnancy outcomes.
In clinical settings, detection of anti-β2GPI IgG is essential for diagnosing APS, especially when present alongside other antiphospholipid antibodies like lupus anticoagulant and anticardiolipin IgG. It is measured using standardized ELISA methods, and persistent positivity (at least 12 weeks apart) confirms its diagnostic relevance. Elevated levels can help stratify patients at higher risk for thrombotic complications, guiding long-term anticoagulation or prophylactic treatment during pregnancy. In research, anti-β2GPI IgG is studied for its role in endothelial cell activation, complement activation, and thrombogenesis. It also serves as a model for investigating autoimmune-mediated vascular injury and the development of targeted therapies that disrupt pathogenic antibody–antigen interactions without broadly suppressing the immune system.
