Ustekinumab is a fully human IgG1κ monoclonal antibody that targets the p40 subunit shared by interleukins IL-12 and IL-23. It acts by inhibiting the IL-12/23 signaling pathway, which plays a critical role in modulating T-cell responses and pro-inflammatory cytokine production. By preventing these cytokines from binding to their respective IL-12Rβ1 receptor subunits on immune cells, ustekinumab downregulates Th1 and Th17 pathways, leading to reduced inflammation and immune cell activation.
Clinically, ustekinumab is classified as a biologic disease-modifying anti-rheumatic drug (bDMARD) and is approved for use in treating chronic inflammatory diseases such as psoriasis and inflammatory bowel disease. Importantly, ustekinumab does not engage in Fc-mediated effector functions like ADCC or CDC, further reducing off-target effects.
