Anti-thyroid peroxidase immunoglobulin G (Anti-TPO IgG) is an autoantibody directed against thyroid peroxidase (TPO), a key enzyme involved in thyroid hormone synthesis. The measurement of anti-TPO IgG, alongside anti-thyroglobulin (anti-Tg) antibodies, is a routine diagnostic tool in the detection and monitoring of autoimmune thyroid diseases. These antibodies are highly correlated with histological changes observed in Hashimoto’s thyroiditis and are often present in a range of thyroid conditions. Clinically, anti-TPO IgG is detected in 70–90% of chronic thyroiditis cases, ~60% of primary hypothyroidism cases, ~50% of thyrotoxicosis, and approximately 17% of thyroid tumors.
The presence of anti-TPO IgG serves as a valuable biomarker in identifying autoimmune activity within the thyroid gland, providing insights into disease progression and aiding in differential diagnosis. While ELISA-based methods are commonly used for detection, background reactivity in healthy individuals has historically limited diagnostic sensitivity and specificity. To address this, advanced ELISA kits—such as the EDI™ high-sensitivity anti-TPO ELISA—utilize proprietary technology to reduce nonspecific background, enhancing both clinical performance and reliability.
In both research and clinical settings, anti-TPO IgG is essential not only for early detection of thyroid dysfunction but also for monitoring therapeutic outcomes and disease progression. Its prevalence in autoimmune thyroid disorders like Hashimoto’s and its correlation with tissue-level damage make it an indispensable marker in endocrine and autoimmune research.
