Anti-Endomysium Antibodies (EmA) are autoantibodies directed against the endomysial component of connective tissue, primarily associated with celiac disease, an autoimmune disorder triggered by gluten ingestion. While IgA EmA is the most commonly measured, IgG EmA can be detected in individuals with IgA deficiency, which occurs in a subset of celiac patients. The presence of IgG EmA reflects an immune response targeting tissue transglutaminase in the small intestine, leading to villous atrophy, malabsorption, and a range of gastrointestinal and extraintestinal symptoms. Detection of these antibodies provides insight into the autoimmune processes underlying gluten sensitivity and intestinal damage.

Celiac EmA IgG ELISA enables sensitive and specific quantification of these autoantibodies in serum or plasma. In research settings, it is used to study the immunopathogenesis of celiac disease, evaluate the prevalence of IgG-mediated responses in IgA-deficient populations, and assess the effects of gluten exposure or experimental therapies on antibody levels. It also helps in investigating links between celiac autoimmunity and other autoimmune disorders.

Clinically, Celiac EmA IgG ELISA serves as an important diagnostic and monitoring tool, particularly for patients with selective IgA deficiency who may test negative for IgA-based celiac antibodies. It aids in the identification of celiac disease in symptomatic individuals, guides decisions on initiating a gluten-free diet, and allows monitoring of antibody titers over time to assess dietary compliance and mucosal healing. This makes it a valuable complement to intestinal biopsy and other serological markers in comprehensive celiac disease management.
