The anti-cardiolipin (aCL) screen is a serologic test used to detect the presence of autoantibodies against cardiolipin, a phospholipid found in mitochondrial membranes and commonly exposed during cellular damage or apoptosis. These antibodies are key components of the antiphospholipid antibody profile and are associated with antiphospholipid syndrome (APS), an autoimmune disorder characterized by recurrent thrombotic events and pregnancy complications. The screen typically detects IgG and IgM isotypes—sometimes IgA—using ELISA, and it serves as an initial step in evaluating patients for APS or related autoimmune conditions like systemic lupus erythematosus (SLE).
In clinical practice, the anti-cardiolipin screen is used when patients present with unexplained blood clots, recurrent miscarriages, or clinical signs suggestive of APS. A positive screen prompts further testing to quantify antibody levels and confirm persistence over 12 weeks, which is essential for meeting classification criteria for APS. In research, the aCL screen helps to explore the prevalence and pathogenic mechanisms of antiphospholipid antibodies in both autoimmune and non-autoimmune populations. It is also used to investigate correlations between antibody titers, clinical severity, and outcomes, as well as to identify novel therapeutic targets for thromboinflammatory processes driven by these antibodies.
