Asymmetric dimethylarginine (ADMA) and L-arginine are key biomarkers involved in the regulation of nitric oxide (NO) production, a critical mediator of vascular structure and function. NO is synthesized in the endothelium from L-arginine by the enzyme nitric oxide synthase (NOS), and plays a vital role in maintaining vascular homeostasis, earning its designation as an “endogenous anti-atherogenic molecule.” ADMA, an endogenous NOS inhibitor, impairs NO synthesis by competing with L-arginine for NOS binding sites, thereby disrupting endothelial function and promoting vascular dysfunction.

Elevated levels of ADMA have been linked to various cardiovascular and metabolic disorders, including hypertension, hypercholesterolemia, chronic heart failure, and renal disease. Clinical studies involving over 10,000 participants have identified high ADMA concentrations as a significant risk factor for cardiovascular events and all-cause mortality. Additionally, the L-arginine/ADMA ratio has emerged as a more accurate indicator of NOS pathway functionality than L-arginine levels alone. This ratio reflects the balance between substrate availability and enzymatic inhibition, making it particularly relevant in both research and clinical settings.

In practice, simultaneous measurement of L-arginine and ADMA plasma concentrations provides valuable insight into endothelial function, vascular risk, and the efficacy of therapeutic interventions, such as nutritional L-arginine supplementation. The inclusion of ADMA and the L-arginine/ADMA ratio as biomarkers supports early disease detection, patient stratification, and monitoring of treatment outcomes in cardiovascular and renal health research.
