Anti-LKM1 (liver-kidney microsomal type 1) antibodies are autoantibodies directed against cytochrome P450 2D6, an enzyme primarily expressed in hepatocytes and, to a lesser extent, in renal tissue. These antibodies are most commonly associated with autoimmune hepatitis type 2, a condition characterized by immune-mediated liver injury that can lead to chronic inflammation, fibrosis, and, in severe cases, cirrhosis. The presence of Anti-LKM1 indicates an autoimmune response specifically targeting hepatic microsomal proteins, reflecting underlying dysregulation of immune tolerance and offering insight into disease mechanisms involving hepatocyte-specific autoimmunity.

The Anti-LKM1 ELISA allows for the quantitative detection of these antibodies in patient serum, providing a sensitive and specific method for identifying autoimmune hepatitis type 2. In research settings, this assay is used to study the pathogenesis of autoimmune liver diseases, investigate the role of B-cell and T-cell responses in autoimmunity, and explore potential triggers or genetic factors contributing to antibody production. It can also be used to monitor experimental models of autoimmune liver injury and evaluate novel immunomodulatory therapies.

Clinically, Anti-LKM1 ELISA is a key diagnostic tool for distinguishing autoimmune hepatitis type 2 from other liver disorders, including viral hepatitis and drug-induced liver injury. Detection of Anti-LKM1 supports early diagnosis, informs prognosis, and helps guide immunosuppressive treatment strategies. Additionally, measuring antibody titers over time can assist clinicians in monitoring disease activity, assessing response to therapy, and predicting potential relapses in patients with autoimmune liver disease.
