Anti–Beta-2 Glycoprotein I (anti-β2GPI) antibodies are autoantibodies that target β2-glycoprotein I, a plasma protein involved in coagulation and phospholipid binding. These antibodies are central to the pathogenesis of antiphospholipid syndrome (APS), an autoimmune disorder characterized by arterial and venous thrombosis, recurrent pregnancy loss, and thrombocytopenia. Anti-β2GPI antibodies, particularly of the IgG and IgM isotypes, induce a prothrombotic state by interacting with endothelial cells, platelets, and complement pathways, promoting clot formation and vascular inflammation.
In the clinical setting, anti-β2GPI testing is used in combination with anticardiolipin antibodies and lupus anticoagulant to diagnose APS according to the revised Sapporo criteria. Persistent positivity on at least two occasions, 12 weeks apart, confirms clinical relevance. Testing is especially important in patients with systemic lupus erythematosus (SLE), unexplained thrombosis, or recurrent pregnancy complications, as it helps guide anticoagulation therapy, risk assessment, and patient management.
In research, anti-β2GPI antibodies are used as biomarkers to study autoimmune thrombophilia, endothelial dysfunction, and pregnancy-related complications. They play a role in mechanistic studies exploring antibody-mediated coagulation, complement activation, and vascular injury, and are employed in longitudinal studies to identify individuals at risk for APS. Additionally, anti-β2GPI measurements are valuable endpoints in clinical trials of anticoagulants or immunomodulatory therapies, providing insights into the immunopathogenesis of thrombotic autoimmune disorders.
