Cortisol is a glucocorticoid hormone produced by the adrenal cortex in response to adrenocorticotropic hormone (ACTH) signaling from the pituitary gland. It plays a central role in regulating metabolism, immune function, stress responses, and circadian rhythms. Cortisol levels fluctuate throughout the day, peaking in the early morning and declining at night, reflecting the body’s natural diurnal rhythm. As a biomarker, cortisol provides insight into hypothalamic–pituitary–adrenal (HPA) axis activity, physiological stress, and endocrine function. It can be measured in serum, plasma, saliva, urine, or hair, depending on whether acute or chronic exposure is being assessed.

In research, cortisol is widely used to study stress physiology, neuroendocrine function, and metabolic regulation. Its measurement helps investigate the effects of acute or chronic stress, sleep patterns, psychological conditions, and pharmacological interventions on HPA axis activity. Researchers also use cortisol to explore links between stress, immune function, and disease susceptibility, including metabolic disorders, cardiovascular disease, and psychiatric conditions such as depression and anxiety.
