Neuron-Specific Enolase (NSE) is a glycolytic enzyme found primarily in neuronal and neuroendocrine cells. It is one of the three isoenzymes of enolase, with the γγ isoform (NSE) being highly specific to neurons and neuroendocrine tissues. NSE plays a key role in glycolysis by catalyzing the conversion of 2-phosphoglycerate to phosphoenolpyruvate, a crucial step in energy metabolism. Due to its localization within neurons, NSE is released into the extracellular space and bloodstream following neuronal injury or cell death, making it a useful biomarker for neuronal damage and neuroendocrine activity.

In clinical settings, NSE is widely used as a diagnostic and prognostic biomarker. Elevated serum or cerebrospinal fluid (CSF) levels of NSE are associated with various conditions involving neuronal injury, such as traumatic brain injury, stroke, hypoxic-ischemic encephalopathy, and epileptic seizures. In oncology, NSE serves as a tumor marker for certain neuroendocrine tumors—particularly small cell lung carcinoma (SCLC), neuroblastoma, and carcinoid tumors—where its concentration correlates with tumor burden and disease progression. Monitoring NSE levels can aid in assessing treatment response and detecting recurrence.

In research contexts, NSE is utilized to investigate neuronal integrity, neurotoxicity, and mechanisms of neurodegeneration. It serves as a reliable marker for neuronal cells in tissue studies and in vitro models, often used to quantify neuronal differentiation or injury in experimental systems. Furthermore, NSE measurements help elucidate pathophysiological processes in central nervous system (CNS) disorders and support the development of neuroprotective or regenerative therapies. Thus, NSE bridges clinical diagnostics and experimental neuroscience by providing insights into both disease pathology and therapeutic outcomes.
