NT-proCNP (N-terminal pro-C-type natriuretic peptide) is a stable peptide fragment derived from the precursor of C-type natriuretic peptide (CNP), a member of the natriuretic peptide family that includes ANP and BNP. Unlike its relatives, which primarily regulate cardiac function, CNP is more strongly associated with vascular homeostasis, bone growth, and endothelial cell function. NT-proCNP is co-secreted in equimolar amounts with active CNP during the cleavage of proCNP and serves as a more reliable biomarker due to its longer half-life and greater plasma stability compared to the active peptide.

In clinical research, NT-proCNP has been investigated as a biomarker of endothelial dysfunction, cardiovascular risk, bone metabolism disorders, and sepsis or systemic inflammation. Elevated levels have been associated with vascular inflammation, heart failure, and impaired bone growth in pediatric populations. In clinical trials, NT-proCNP is also used to monitor treatment response or disease progression in conditions affecting the vascular endothelium or skeletal system.

In basic and translational research, NT-proCNP is used to study the regulation of CNP signaling, particularly in relation to nitric oxide pathways, endothelial integrity, and fibroblast growth factor signaling. It is also valuable in the development of therapies targeting vascular repair, hypertension, and osteochondral pathologies. Due to its measurable stability in serum or plasma, NT-proCNP is an ideal surrogate for CNP production in both human and animal models.
